User Process

1.   The cardholder must notify the call agent or the counter nurse that they will be paying with UnionPay during their appointment or walk-in visit

2.   The cardholder requests the applicable discount before completing the payment

4.   If a patient requires a referral to an ophthalmologist, a separate appointment must be scheduled, and additional charges will apply

5.   Contact lens wearers must remove their lenses for 1 to 3 weeks before the eye examination. (Cardholders should reconfirm the required duration with OPTIMAX when scheduling an appointment.)

6.   A registration fee applies to new patients

7.   This offer cannot be combined with other promotions, discounts, referrals, or vouchers and is not exchangeable for cash
